Sadly, many people impacted by advanced illness and frailty report feeling lonely and socially isolated.
Loneliness and social isolation negatively affects a person’s mental, emotional and physical health.
The damage to health is widely accepted to be equivalent to smoking 15 cigarettes a day.

End of life is part of life.
Our commitment is to co-design a compassionate community response that will support people on their final journey, and the continued journey for those they leave behind.
Living well to the end of life.
Our commitment is to normalise conversations around death and to improve experiences of dying, death and bereavement.

A compassionate response to the challenges regarding end of life is relevant to all communities.

CCNI operates from two hubs in Northern Ireland. Located in the North West and Northern localities.
