Shaping a Healthcare System in Partnership with Our Communities



Healthcare in Northern Ireland is changing. To build a system that truly serves local needs, we must move away from a model that simply delivers services to people, and start designing care with our communities as active, equal partners.

As part of the Health and Social Care (HSC) Reset Plan, the Public Health Agency (PHA) and the Patient & Client Council (PCC) have launched This is Our Health This initiative is a direct commitment to engage with local areas, tackle health inequalities, and shift the culture toward community-oriented care.

Compassionate Communities NI advocates for improving the experiences of people impacted by advanced illness, caregiving, end of life and bereavement. We want to

ensure that the vital voices of those navigating these difficult life transitions are at the heart of our local healthcare design.

If you have lived experience as a patient, caregiver, or through bereavement, your insights are essential to building strong social infrastructure and proactive, dignity-first services right where you live.
